As a shellfish aquaculture leaseholder in the state of Maryland, you are required to report your monthly harvest. Once you return your Shellfish Aquaculture Harvester Permit (SAHP) Registration form to the Department, you will receive everything needed to report your monthly harvest in your SAHP packet. This includes Maryland Monthly Shellfish Aquaculture Harvest Report forms, pre-addressed envelopes and barcodes with your name, lease number, and month harvested printed on the stickers. You will also receive your Shellfish Aquaculture Harvester Permit(s) in this packet. The number of blank report forms mailed in your packet will depend on when your Harvester Permits were issued. Those who received permits later in the year were sent forms, envelopes and barcodes for the remainder of the calendar year. You are required to submit a report for a partially permitted month, even if you were issued a permit on the last day of that month.
Monthly Harvest Reporting - Video Tutorial
You must complete a Maryland Monthly Shellfish Aquaculture Harvest Report in its entirety for each month of the year. This means all information must be complete and accurate for every day of shellfish harvest. If you did not harvest any shellfish in a month, you are still required to complete the form indicating "NO HARVEST", sign, and submit to the Department.
The reports should be mailed to: Department of Natural Resources, Fishing and Boating Services, Shellfish Aquaculture Harvest Report, 580 Taylor Avenue B-2, Annapolis, MD 21401 in the pre-addressed envelope with corresponding barcode(s) attached, or faxed to: 410-260-8279.
Reports must be received by the 10th day of the month immediately following the month the harvest occurred. Failure to submit your reports in a timely manner may affect the standing of your lease with the Department.
If you are unsure if you are missing a report, please contact the Aquaculture Division at 410-260-2405 to inquire about your status. Shellfish Aquaculture Harvester Permit renewal for the next year may be delayed for those leases which are still missing reports at the end of the year.

The National Shellfish Sanitation Program (NSSP) Model Ordinance dictates the safe handling of shellfish for human consumption. In order to comply with the requirements of the NSSP and decrease the risk of illness, Maryland has developed a Vibrio parahaemolyticus (V.p.) Control Plan. V. parahaemolyticus illness is caused by the consumption of raw molluscan shellfish (clams and oysters) infected with this bacterium. Maryland's Vibrio parahaemolyticus Control Plan requires leaseholders engaged in the summertime harvest of shellfish to implement certain control measures in accordance with the Code of Maryland Regulations (COMAR) 10.15.07.06 and 08.02.23.04(F) 6.
From June 1 through September 30, a Shellfish Aquaculture Harvester Permittee or Registrant acting under the authority of the permittee is required to:
To comply with item 3 above, from June 1 through September 30 you must land your product at a SINGLE site and provide the location of your SINGLE landing site to the Department of Natural Resources. Please list the landing site below. If no harvest will take place, please indicate that by checking the box and SIGNING the form.
