ANNUAL USED REGULAR Annual leave used following the policies that govern use thereof.
11 ANNUAL USED SICK DOCUMENTED Annual leave used for a documented sick occurrence. 12 ANNUAL / SHORT NOTICE Annual leave used without prior approval. 13 ANNUAL/FMLA Annual leave used in accordance with the FAMILY MEDICAL LEAVE ACT 20 SICK USED DOCUMENTED Sick leave used for a documented illness. 21 SICK USED UNDOCUMENTED Sick leave used for an undocumented illness. 22 SICK HEALTH CARE Sick leave used for a doctor’s appointment–must have a doctor note. 23 SICK DEATH IN FAMILY Sick leave used as a result of the death of a family relative. 25 BEREAVEMENT LEAVE Administrative leave used in lieu of sick leave for death in the employee’s “immediate” family only, Maximum three days. COMAR 17.04.11.06 26 SICK/FAMILY DOCUMENTED Sick leave used when a member of the immediate family is ill, and the employee documents that usage with a medical certificate. 27 SICK / FAMILY UNDOCUMENTED Sick leave used when a member of the immediate family is ill, and the period of sick leave is not documented. 28 SICK – ADOPTION DOCUMENTED Sick leave used by employee after the adoption of a child, documentation required. 29 LEAVE BANK Code used for an employee granted leave from the leave bank maintained by DBM. 30 SICK CHRONIC Used for sick leave usage that occurs subsequent to a documented illness-injury-chronic condition. Requires medical documentation that states the usage is directly related to the initial illness-injury-chronic condition. 32 ORGAN DONATION Leave taken for donating an organ. 33 SICK FMLA Sick leave taken in accordance with Family and Medical Leave Act. Medical Documentation Required. 35 ACCIDENT LEAVE FMLA Accident Sick leave taken in accordance with Family and Medical Leave Act. Requires medical documentation and approval by Appointing Authority. 36 DONATED FMLA Donated leave taken in accordance with Family and Medical Leave Act. Medical documentation required 38 DONATED LEAVE USED Code used when an employee uses donated leave from another employee through employee to employee donated leave program. 39 ACCIDENT LEAVE Leave with pay to an employee who sustains an accidental injury while in the actual performance of duties. 40 COMP EARNED Code used when employee has been approved to work in excess of normal work hours, and in accordance with agency compensatory policy. This code will be used by employees who are required to work on a State Holiday also. This does not include employees working 28-day schedule. 41 COMP USED This code is used when an employee is authorized the use of earned compensatory time. 42 COMP USED/SICK DOC. This code is to be used when an employee is absent from work due to a documented illness and wishes to use earned comp time. 43 COMP USED SICK–COE COE comp used for sickness. 45 COMP TIME RELIGIOUS Code used when employee has been approved to work in excess of normal work hours to earn comp time to be used for a religious observance. Religious holiday and date to be used must be written on time sheet/ comp time must be used on that date, or it is lost. 46 COMP USED RELIGIOUS This code is used when an employee uses earned religious compensatory time for a religious observance. 47 DNR/LEO/PCO THREE HOLIDAY RULING DNR Law Enforcement and Police Communications Operators who are HOLIDAY RULING required to work on New Years, Thanksgiving, or Christmas Days, should Use this code when the leave is earned. Code 41 should be used when the Holiday comp time is approved to be taken. Must be taken within one year from the date that it is earned. 48 COMP EARN COE CASH OVERTIME ELIGIBLE) Comp time earned by an employee who is eligible to receive cash overtime pay, but chooses compensatory time instead. 49 COMP USED COE Comp time used which had been earned by a cash overtime eligible employee. 50 PERSONAL REGULAR This code is used to record personal leave used. 51 PERSONAL SICK DOC. Code used when personal leave is used for a documented sick occurrence. 52 PERSONAL UNSCHEDULED Code used to record Personal leave used that was not scheduled in advance. 53 PERSONAL FMLA Personal leave taken in accordance with Family and Medical Leave Act. Medical documentation is required. 60 LWOP PERSONAL Code used when an employee is authorized time off from work, and is not paid for the time. No deduction is made from any leave balance, nor is any time accrued for the period. 61 LWOP SICK DOC. Code used when an employee takes off from work because of a documented sickness, and is not paid for the time. 62 LWOP MILITARY Code used for an employee who enters the armed forces and who wished to take a leave of absence without pay. 63 LWOP FMLA Unpaid leave taken in accordance with Family and Medical Leave Act. Medical documentation is required. 64 LWOP SEASONAL Code used for an employee who is granted seasonal leave. 65 LWOP UNAUTHORIZED Code used when no approved leave or leave without pay is authorized. 66 LWOP DISP. SUSP. Code used for leave without pay for disciplinary suspensions. 70 MILITARY Code used for an employee who is a member of the Military Reserves who is absent for military training. (Fifteen days maximum per year) 71 MERIT INTERVIEW EXAM Code used when an employee documents taking merit examinations, or attending state interviews only. (Up to four hours allowed.) 72 EMERGENCY RELEASE Code used when an employee is released from work without charge to any leave under emergency conditions. This is paid leave. 73 JURY Code used for an employee who documents serving as a member of any jury. This is paid leave and is not charged to any other paid leave. 74 LEGAL ACTION Code used for an employee who documents a summons to appear in any court action, before a Grand Jury, or an Administrative Agency, etc., and is not a party or paid witness. 75 GRIEVANCE Code used for an employee who documents he is a party to, or is called as a witness in a Grievance, and the time is directly related to the Grievance. This is paid leave. 76 EMP. ORG. EVENT Code used for the time off from work Authorized to an employee to attend an employee organization event. 77 TRAINING This code is used to record the time spent in various training activities. This includes seminars and training courses. 78 ADMINISTRATIVE LEAVE This is time off with pay awarded by an appointing authority. 79 ADM PENDING DIS. Administrative leave authorized by agency during investigation for disciplinary purposes. 80 DNR LAW ENFORCEMENT Schedule “off” days for those law enforcement employees who work a twenty-eight day schedule. 82 DISASTER LEAVE Used for leave available to Red Cross certified disaster service volunteers. Limited to 15 days in a 12 month period. Supervisory approval is required. Level II or above disaster. 89 MILITRY LEAVE BANK Used for Military Leave Bank 98 DNR LAW ENFORCEMENT POLICE COMM. OPERATORS Used for Holidays taken at 24 hour institutions that don’t follow the State Holiday calendar. 99 HOLIDAY Used when an employee is absent on legal holidays.
